• Proposed revised IWO form was out for public comment - DCL 07-05

• Child Name and Date of Birth - moved to front of the form
• Lump Sum Payments - clearly indicates that form may be used for lump sum payments

• Termination of income withholding - separate page modified to include relevant employee information to allow submission as a stand-alone document
